Student consent form
Form version: v1
Why do we need this form?
Science Tutoring keeps your child's name, class, date of birth and mobile number to run its services. Today these are the NTA Test Simulator and Doubt Solver. A new Indian law (the DPDP Act) says that for children under 18, a parent or guardian must give permission first. This form is that permission.
We use the details only to run your account. We do not sell them. They are kept with Science Tutoring while you are a student, plus 6 months, then deleted. A parent can withdraw permission any time by contacting Sayak Sir on 9831917170. In Doubt Solver, questions go to outside AI services, so please do not type personal details in a question.
You give permission for each service separately. When Science Tutoring starts a new service, you will be asked again for that service.
Step 1. What is this form for?
Tick every service the student will use.
Step 2. Download, print, sign
- Download the form.
- Print it and fill it by hand.
- Tick the same services here and on the form.
- Sign it. Under 18? A parent or guardian signs.
- Take a clear photo of the full page. All four corners should be visible and the writing readable.
You can print it, or use a pen on a tablet. A mobile number is a must. For Doubt Solver the student's Google email is a must too.
What happens next
Sayak Sir checks your form and keeps it safe with Science Tutoring. If you ticked the Test Simulator, he gives the student a login ID and password. This may take a day. If you ticked Doubt Solver, access opens as soon as the upload says Done, with the Google email you typed. Questions? Contact Sayak Sir on 9831917170.
