What is this? Before Sayak Sir gives a student a login ID and password for the NTA Test Simulator, he needs a signed consent form. This page is for students who cannot come in person.

4. Sign it. Under 18? A parent or guardian signs.
5. Take a clear photo of the full page.
6. Upload the photo on this page.

Mobile number is a must. We cannot issue a login ID without it. We use it only to contact you about this account.

Your data: We keep the name, date of birth, class, mobile number and form photo only to give and manage the student's account. Only Sayak Sir can see it. We keep it while the subscription is active and for 6 months after it ends, then delete it. To see, correct or delete it, contact Sayak Sir on 9831917170.

You can print it, or use a pen on a tablet.

Step 2. Fill and sign

Write the student name, date of birth, class and mobile number. Tick one box. Sign. Take a clear photo or scan of the full page. All four corners should be visible, and the writing should be readable.

What happens next
